Your first visit is your opportunity to get to know our practice, see our facility, and meet our office team. During your initial appointment, we’ll discuss your dental concerns, collect the relevant health history and background information, and perform a clinical exam to establish a baseline for care.
We strive to ensure your first visit with us is comfortable and personal acknowledging your personal dental needs. On your first visit, we will take the time to properly diagnose your immediate dental concerns, review your past medical and dental history, and create a treatment plan that provides for your optimal dental health. This includes an examination and review of your treatment options, scheduling an appointment for your dental needs.

You can save time and expedite your first visit by downloading and completing the forms in our “New Patient Package” at home. Simply hand the forms to our receptionist at check-in. Also, remember to bring your insurance card and an ID with you to your first appointment.
In an effort to avoid any misunderstandings, we would like to review our financial and office policies before you begin treatment in our office.
Payment is expected at the time services are performed. We accept cash, check, and all major credit/debit cards. For extensive services we offer low and no interest payment plans through Care credit.
You will need to supply us with your dental insurance information (name, date of birth, social security number, employer, and dental ID #). We will do our best to answer any questions you may have about your benefits, but always suggest that you call or visit your insurance company’s website.
We will collect any required estimated co-payment and deductible at each visit. We make every effort to determine your benefits when you receive treatment, but consider your co-payment an estimate until we receive payment from your insurance company.

We reserve appointment times specifically for each patient so that we may provide the ultimate care in service. Please schedule your appointment carefully as there will be a fee of $50 for hygiene appointments, and a fee of $100 for the doctors appointments. for any appointment missed or cancelled without 48 hours notice. Similarly, late arrivals can create scheduling problems. Please notify us if you are going to be late for any reason whatsoever.

We accept Mastercard, Visa, American Express and Discover credit cards as well as cash and personal checks. We accept most major PPO insurance plans. In addition, financing options such as CareCredit, Sunbit and our in-house savings plan are available for major dental work done in our office. We will be happy to discuss any insurance or billing questions you may have.
